Attendance Policies

The form below is for families to report (not excuse) student absences, tardies, check out early, or return to campus from scheduled appointments.

Este formulario es para que las familias informen (no excusen) las ausencias, tardanzas, salidas temprano o regresar al campus de la cita programada de los estudiantes.

Attendance form

Please leave the following information:

1. State and spell the student's name.
2. Date of absence.
3. Reason for absence (illness, medical appointment, etc.)
4. Caller name, phone number, and relationship to the student.

A call or submitted form must be made for each day that a student will be absent.
